The Responsibility Must Fall to the Administrators


I believe we must work more on service consumer protection. I don´t think consumer protection is, as we constantly hear, just the right to claim, because people claim and claim and the best possible result is a concession they are right. For example, every time I go to Coppelia, [very popular and central ice-cream parlor in Havana] I know beforehand that when they bring me my ice cream I’ll return it – I always do – and they bring back a better served plate; I don’t know if this second plate complies with the norm because there is no way to tell, but you can see the change. When I go to the agro market and there is a scale for the use of consumers, I check the weight of my purchases, and if I claim, they give me the difference; they seldom argue, because they know they are stealing. It is a fact that the solution is not in the inspectors, because they have been there for a long time now and things remain the same.

I think the responsibility for this situation falls on the administrators. As long as they take the side of their employees against the consumers, the problems won’t be solved. Many times the claims are made in the presence of the administrator and the employee refunds what was missing, and that’s all, nothing else happens; they see the problem as a legitimate claim that got sorted out; they don´t see it as one of his employees stealing from the customer.

A good administrator must be outside his office most of the time, watching that their customers are well treated, that the norms are applied and checking that his employees are not stealing from the customer. If he has to penalize someone, the punishment should be recorded in the worker’s file. At present they fire the worker and he or she goes to work somewhere else and keeps doing the same. Inspectors should do more than give out fines, they need to follow up on the penalizations and find out what the administrator has done; in other words, find a way to make the greatest demand on the administrator, not the employee. There should be more surprise visits – as journalists do for their reports and always find many problems – and exemplary discipline actions must be taken.   

If the scales are not properly calibrated, if the price of the product is not right, the responsibility falls on the administrator, not the employee; if the ice cream balls don’t have the right size – this does not happen in Coppelia alone – the responsibility must fall on the administrator or the shift supervisor, not on the employee who serves the ice-cream. The same in restaurants where menus list the grams of each serving, but you have no way to check it and they often serve you less.   

The solution is to create the necessary procedures of control, so that the administrator feels compelled to do his duty and understands he is responsible for all the problems; not as it is now when they are seldom there or couldn’t care less about the problems. You go to a bakery and see there is a consumer protection committee and the first name in the list is the administrator’s; there the buns do not have the required weigh, the sweets have poor quality and you think, “Who can I make my claim to, if all the products were made in his watch?”
Of course, in order for all good things to work, we need to make legitimate job compensation attractive for both administrators and employees, based on the results of their work; otherwise administrators cannot easily find a substitute for any employee they need to fire. Employees may also leave if pressure is exerted on administrators so they cannot connive with their employees.

When There Is a Poorly Planned Economy…

Paraphrasing the heading of Ronald Suárez Rivas’ report of Friday, July 15th, I’d like to show you the other side of the coin.

All my childhood and most of my teenage years I lived in Santa Damiana, part of the Río Seco Community, in San Juan y Martínez municipality. The same drinking water wells have existed for time immemorial (2) Who paid for this?  At meetings of the now extinct CPA Antonio Guiteras Holmes [CPA Cooperativa de Producción Azucarera – Sugar Producers Cooperative] or of the CCS Francisco Blanco, [
CCS Cooperativa de Crédito y Servicios – Credit and Services Cooperative] the topic was discussed, but since there was “no one” to foot the bill of the fuel needed to pump the drinking water for the population, everything remained the same. Nationwide, in many rural areas in the country there must be similar situations.

The people in my hometown get water twice a week. My parents still live there and their water supply comes Tuesdays and Saturdays, provided there is fuel or the pumps are not broken. Now, if the “poorly planned and wasteful consumption of fuel is painful” – in my neighborhood about 1300 liters – much worse is the loss of water which occurs every time such pumps are turned on. My calculation is of a loss of 60% to 80% of the drinking water pumped. This is due to leakage and broken sections in a pipeline with more than 30 years of use without maintenance or repairs from any entity or cooperative.

And I say this with forewarning on my mind, lest the solution is to “get rid of the sofa” as in the story of the cuckold husband. Just in case someone thinks the solution to this “poor economy planning” is to stop the supply of water to the population.
If our present economic situation were similar to that in the late 70’s or 80’s of last century, the pipelines would be fixed or substituted as they are crying -- with gushes of water -- for repairs, but, Who will pay for the investment? 

Another topic I wish to discuss: It is infuriating to read that municipal agriculture and CCS´s directors say that jobs such as filling up tobacco plantations with organic nutrients cannot be done because there is a lack of diesel. In the years when my father owned his land, during our holiday months, my brothers and my father did lots and lots of hand shoveling of nutrient-filler from ditches, streams, and banks of rivers, with a simple cart, one or two shovels, an old and worn cucharón (a farmers’ tool now extinct in the fields of my birthplace), a team of oxen and a real desire to do things the way they should be done. Today you walk along the lands of my zone and see there´s very few plots which get this very important element to restore the organic matter lost to soil erosion. Many tobacco plantations are a meter below the surrounding roads and paths, each day more deprived of organic matter and richer in solid stones. No tractors, or carts, or fuel expenditures.

Let´s retake the traditional use of animal traction and see the results. Let’s provide material compensation to the farmers who rescue extinct traditional techniques. Land gives us fruits and results, but let’s contribute to the process, not only with inorganic and chemical fertilizers, but also with those Mother Earth gives us. The painful time will come when she will not be able to provide for us.   
What I have expressed here has the sole purpose of increasing awareness among State entities, farmer organizations and cooperatives so they do what they need to do: use properly the resources given to them and follow the technical and agricultural procedures and norms they frequently ignore.

And I close with a question: Who will now supply the water to my parents?

Extending the right to have an opinion on everyday life issues

Many important steps are being taken to consolidate the achievements of the Revolution and ratify its Socialist nature. The words of our leaders move to abandon old norms and formulas that presently hinder and obstruct our economy and life in general. The practice of too many meetings and the passivity of many officials are more than obvious but, despite the summons to change, most of the times we attend the meetings to listen to the same words and the same unfulfilled commitments.
The words of Comrade Machado Ventura call for a permanent eradication of these problems. Things are changing in our blockaded Island and people feel it in the environment, even when not at the pace and in the way many of us wish.

Our President, Raul Castro, at the recently concluded 2nd. Plenary Meeting of the Central Committee of the Party emphasized the idea that any discrepancy within the Permanent Committee for Implementation and Development would be analyzed before rejection, thus guaranteeing a free debate and the provision of opportunities for everyone to express its opinion and desires. As he said later at the National Assembly, this concept is extended to the everyday life of Cuban citizens so they have a space to exercise their right to express their opinion with the right words and due respect.

I join those who ask the Mesas Redondas should deal more with national issues and present views and opinions on these issues.
There are people with a lot to say, with many ideas to contribute, who deserve to and should be heard. We should not fear judgment or fair criticism if these are constructive and civil; and if they offer solutions, much better.

It’s imperative to create a mechanism, with no commitments but to truth and justice, to channel the everyday claims of the population about ill-treatments from officials in positions created to serve the people. Unfortunately, and I say this from my own experience, many of these complaints get lost in complacency and indolence. It’s irritating and painful that cases only have a quick response when they are published in the national press, frequently after the letter senders had gone everywhere and done everything they could for months, and sometimes for years seeking a solution to their problems. Most of the times the chiefs became aware of the situations (many of which waited for their signature) only when the whole of Cuba knew about it. Unfortunately, there are many cadres who stare idle at situations whose solution does not fall from above.

We can all contribute to what the country and the people need right now. We must all be heard. Sometimes trenches of ideas are stronger than trenches of stone.

Can we put an end to indiscipline… or can’t we?

Over two years now reading opinions on how to make amends and change whatever harms us into options that would stimulate our economy for our common welfare and rescue the main objectives for which we have struggled in different ways and places, but we don´t see materialize as quickly as we wish and with the quality we hope for. It’s been dragging for years.
The credibility of some institutions and officials is alarmingly weaker each day and we must do something about it.

A permanent inventory of the problems must be kept (the different Ministries and the Poder Popular need to do this) and nip in the bud every deviation from ethical  behavior (immoral practices), because we cannot wait until pigs fly, or  find out at conferences, assemblies, meetings, or from a comment in our press or, worse, in the foreign press.

Just like in a warehouse with the adequate organization the problem is not difficult to discover, and it must be punished with severity. How long must we wait?
The cadre who cannot take this responsibility and be accountable, cannot hold the position (at whichever decision-making level he is). Somebody must be able to do this; otherwise, what solutions are we talking about?

Can we put an end to indiscipline… or can’t we?

Every ministry knows, because there is legislation -- laws and regulations – that spells out what should be done and what is wrong. Those in any managerial position who ignore this are not qualified, and may be wrongly permissive. When someone becomes obsolete (just like spare parts or supplies) it needs to be removed from the list of useful items.
This is what we must do with harmful persistent atrocities. Continuity is of the essence. Yes, day after day. How? What about the good cadres and administrative staff? But, beware: the management ladder must not have any rotten steps, because it is contagious and all good wishes go up in smoke. Will we have what we need to have?  Those who don’t must step down. In our ranks there are responsible and qualified people willing to undertake tasks and make sacrifices.

We must save Socialism. We have to do the right thing for Fidel and Raul. Are all the so called cadres, both political and administrative, real cadres? I believe many are just standing in the way, opportunistically defending their fringe benefits and, therefore, they must go. Whoever stands in the way must go. Where to? To wherever he can learn, or pay for his apathy or negligence.

Finally, in relation to inventories, let’s not underestimate grass roots operative positions where some employees may seek what could be in there for them and take the salary just as a secondary source of income, because this is less than what they can make by stealing.

Seldom do we find a service or production posting where these things do not happen. Ours is a Revolution of the meek, with the meek and for the meek.

President Raul has already taken some measures and his foot is on the gas rather than on the brakes. His support staff (at all levels) should rise to the occasion and reach the population even more. This is the only true way. The task is huge, but we must always remember the encounter at Cinco Palmas.

Let´s improve our health system with a better attitude. We can.

A few years ago, a doctor friend who practices at the 10 de Octubre Hospital (still popularly known as La Dependiente) told me to go see him at his clinic. He said I should go around noon and explained that at 12:00 he had finished seeing all the patients who had appointments. Many years have gone by and they have taken along a bit of the discipline that should characterize access to a doctor’s clinic. My doctor friend believed his friends should not take priority over all the patients who arrived early to the clinic. For him, all his patients were also his friends; he respects them and in turn the patients feel respect and consideration for him.

This introduction allows me to point out that the waiting lines to see doctors anywhere -- at the family doctor’s, the polyclinics or the hospitals -- have become disorganized; quite often, the friends of the staff or the people who bring the best presents go first. Let me clarify that I’m not against giving presents to doctors or nurses, but this should not make a difference in terms of what patient is seen first. For any specialty, in any clinic, there is disorder, firstly because the assisting nurse or the ward secretary does not take the responsibility of writing down the names of the patients as they arrive to the clinic and of telling each one they must wait until they are called.

 Every person that goes to a clinic does so because he or she has an ailment. They don’t go for no reason. It’s known that sometimes we go to the emergency room of the polyclinic or the hospital because there is no doctor at the family doctor’s clinic or because he or she arrives at unpredictable times, or leave early for a number of reasons legitimate or not. Sometimes they arrive late because the municipal authority scheduled a meeting or a task other than the primary task of a family doctor which is – and has been since the system was created -- to care for the families. Compañero Fidel identified them as the 120 family clinics. Today in fact each clinic has more than 120 families under its care. It’s not the family doctor’s fault that he is given other tasks and he feels bad when he hears the complaints of the population in his care, because his arguments are questionable.

Another problem: Prescription drugs and other pharmaceutical products are supplied to the drugstore, let’s say in the morning, and then, by magic, they are no longer available in the afternoon. What a mystery! Some disappear, and others are hoarded by unscrupulous unlicensed traders who act with impunity.

A few more few comments:
--Discipline and organization at the clinic are tasks of the doctor, the nurse and the ward or specialty secretary. This is the public opinion one hears every day. Unfortunately, I attend clinics quite frequently and have had the opportunity to tell apart what is good and what doesn’t work well. 

--We can’t understand, -- and there is no reasonable explanation – why there are no prescription forms at the clinics. Sometimes a doctor is given only 10, 20, or 30 forms, as if someone could plan beforehand how many patients would need so many prescriptions.

--Very few doctors perform clinical examinations of their patients and, quite often without second thoughts prescribe CAT scans or MRI. Sometimes they barely listen to the patient and prescribe a certain medication; then when the patient returns in worse shape they are no longer there. This happens sometimes because they were having a chat on the phone or with a visiting friend and conversation took priority. This shows lack of medical ethics on the part of some doctors.     

--This happens frequently: the doctor allows several people in the room; you enter the room planning to discuss a few private medical things, or ask a few very private questions; and you leave without telling the doctor the truth or avoiding certain questions. The doctor needs to guarantee the patients’ privacy.

--In some places it is urgent to create better working conditions for the doctors. They should be offered a better lunch or have a one-session working day.

--A doctor who spent the night on duty is not fit to keep working and see patients the next morning. I’ve seen them yawning while attending to children. Who can guarantee he could really focus on the child’s health? Can an accurate diagnosis be ensured?

--Very rarely do we see a clinic open in the afternoon. Patients are sent from everywhere to the clinics as if they were open and the doctor were there any working day at any working hour. 

--If  today we don’t provide maintenance and repair to the facilities for primary health care, tomorrow there will be fewer rescue possibilities because the expenses will be higher and our economy cannot take it. 
We know MINSAP [Ministry for Public Health] is working on this and efforts are made to increase the quality of the service. In last week’s Mesa Redonda we could see how much is being done in the country to improve health services. In many places in the world they have all the resources and highly qualified personnel but they don’t reach our levels of medical care, because they lack the heart, they just prioritize profits.

--In Cuba, in our present economic situation with our levels of scarcity, we know very well what is priceless: that is, when the human factor is prioritized, and care is taken of both the medical staff and the patients who receive the service and perceive its quality and a determination to make them feel better and happier. Here, in health services, we don’t work for money, and we don’t ignore salaries are insufficient.  A smile always reaches the heart.

--There is no point in having a well equipped health facility if work there is not done with the only purpose of contributing to the betterment of human health. Let’s remain being a well known world medical power. Let’s improve our health services with a better attitude. We can, and we know we can.

Los principales responsables son los administradores

Creo que en uno de los aspectos en que más hay que trabajar es en la protección al consumidor en los servicios. No creo que la protección al consumidor sea solo el derecho a reclamar como tanto se dice, porque la gente se cansa de reclamar y lo único que pasa, en el mejor de los casos, es que te den la razón; por ejemplo, cada vez que voy a Coppelia sé que cuando me traigan las bolas de helado las voy a virar, siempre lo hago y siempre me traen el plato mejor servido, no sé si es lo que está normado porque no hay forma de comprobarlo, pero se nota el cambio; cuando voy al agro y hay pesa de comprobación, compruebo el peso y si reclamo, me reponen el peso, o sea, casi nunca te discuten porque saben que están robando. Está comprobado que la solución no está en los inspectores, ya que hace tiempo están ahí y la cosa sique igual.

Considero que los principales responsables de la situación son los administradores, mientras se sigan solidarizando más con sus trabajadores que con la población, no se van a resolver los problemas. Muchas veces se hacen las reclamaciones delante de los administradores y el dependiente repone el faltante y no pasa nada, solo lo ven como un acto de reclamación justo, no lo ven como que su empleado le está robando al cliente.

Un buen administrador debe estar gran parte del tiempo fuera de su local, velando por el buen trato y que se cumpla lo establecido, comprobar si sus empleados no le están robando al cliente, y cuando tenga que sancionar que las sanciones aparezcan en los expedientes laborales; en la actualidad, por lo general lo botan y luego van para otro lugar, y siguen haciendo lo mismo. Los inspectores no solo deben poner multas, sino llevar un récord de las sanciones que se han puesto para después verificar qué medida ha tomado el administrador, en fin, buscar la forma para que la exigencia mayor sea sobre el administrador, no sobre el dependiente; se deben hacer más controles sorpresivos a las entidades —como muchas veces han hecho los periodistas en sus reportajes y siempre encuentran muchos problemas—, y aplicar sanciones ejemplarizantes.

Si las pesas están mal calibradas, si el precio del producto no corresponde con la calidad, la responsabilidad es del administrador no del dependiente; si las bolas de helado —porque no es solo en Coppelia— no tienen el tamaño normado, la responsabilidad debe ser del administrador o jefe de área, no del dependiente o el bolero. Lo mismo en los restaurantes, que en la carta dice la cantidad de gramos de las raciones, pero como no puedes comprobarla, muchas veces te sirven de menos.

En fin, la solución está en crear los mecanismos necesarios, ya sea de control o de cualquier tipo, para que el administrador se sienta presionado a cumplir sus funciones y sepa que es el máximo responsable de todos los problemas, que no pase como ahora que casi nunca están o no se interesan por los problemas. Usted va a una panadería y ve que dice comité de protección al consumidor y el primero que aparece es el administrador, entonces, los panes no tienen el peso requerido, los dulces con mala calidad y uno dice, a quién voy a reclamar si todo está hecho a la cara del administrador. Claro está que para que todo esto funcione se debe trabajar para que la remuneración, tanto del dependiente como del administrador, sea atractiva a partir de los resultados del trabajo, ya que si no al administrador le cuesta trabajo conseguir un sustituto cuando bote a alguien, sin contar que se pueden ir sin que los boten cuando le empiecen a exigir al administrador para que no caiga en soborno con los dependientes.

Lo que pasa cuando hay una economía mal planificada

Parodiando el título del reportaje de Ronald Suárez Rivas, publicado el viernes 15 de julio, quisiera hacerles llegar la otra cara de la moneda de lo expresado.

Toda mi infancia y parte de la adolescencia las viví en el barrio Santa Damiana, del Consejo Popular Río Seco, municipio de San Juan y Martínez. Desde siempre han existido los mimos pozos de agua potable (2). ¿Quién asumía el gasto de esto? En las reuniones de la extinta CPA Antonio Guiteras Holmes o de la propia CCS Francisco Blanco, frecuentemente se trataba el tema, pero al "no existir" quien asumiera el gasto de combustible en el suministro de agua potable a la población (Acueducto), todo seguía igual. Nacionalmente, en muchas zonas rurales del país debe existir igual situación.

A la población de dicho lugar se le suministra agua dos veces por semana. Allí viven mis padres todavía y el suministro de agua se efectúa los martes y sábados, siempre y cuando haya combustible o los motores no estén rotos. Pero si "doloroso es el gasto indebido y mal planificado de petróleo, que en mi barrio asciende a 1 300 litros", peor es la pérdida de agua que ocurre cada vez que se ponen a funcionar dichos motores. Calculo que sea entre un 60-80 % la cantidad de agua potable bombeada que se derrocha por salideros y roturas de las tuberías con una vida de uso de más de 30 años, sin ser reparadas por entidad alguna ni cooperativa.

Todo esto "poniendo el parche antes de que surja el ponche" o que se tome la solución análoga al cuento de marras "quemar el sofá", no vaya a ser que la solución a esta "mala planificación de la economía" sea suprimir el abasto del preciado líquido a la población.

De tener en la actualidad una situación económica parecida a la vivida a finales de los 70 o los 80 del pasado siglo, se repara-rían o sustituirían las conductoras de agua, las que actualmente a grandes "gritos" de chorros de agua están pidiendo se les hagan estos arreglos.

Pero, ¿quién asumiría esta inversión?

Otro tema al que quiero referirme: es indignante leer que directivos de la Agricultura municipal o de las CCS referidas digan que tareas como el relleno de las vegas con materia orgánica no se realicen por falta de diesel. En los años en que mi padre era propietario de sus tierras, en los meses de nuestras vacaciones, mis hermanos junto a él bastante "abono-relleno" palearon de las cunetas, arroyos y orillas del río con un simple carretón, alguna que otra pala, un viejo y usado cucharón (instrumento de uso campesino hoy extinto en los campos de mi barrio), una yunta de bueyes y muchísimos deseos de hacer las cosas como tienen que ser. Ahora caminas las tierras de mi zona y ves que son poquísimas las que reciben este importante elemento reconstituyente de la materia orgánica perdida por la erosión de los suelos. Muchísimas vegas están a un metro por debajo de los caminos y trillos que las circunvalan, cada vez más pobres de materia orgánica y cada vez más ricas en piedra viva. Nada de uso de tractores con carretas y el consabido gasto de combustible.

Retomemos el uso tradicional de la fuerza animal y veremos los resultados. Estimulen materialmente a los campesinos que usen estas técnicas tradicionales y ya extintas. La tierra nos da frutos y resultados, pero ayudémosla, no con fertilizantes inorgánicos y químicos, sino con lo que la propia madre natura nos pone a nuestro alcance. Llegará el doloroso momento de que ya no pueda darnos nada más.

Con lo expresado quiero solo llamar la atención a las entidades estatales, campesinas y cooperativistas para que tomen las medidas pertinentes, tanto en el uso correcto de los bienes puestos en sus manos, como en el correcto cumplimiento de normas técnicas agronómicas caídas en el olvido.

Y concluyo con una pregunta: ¿y el agua a mis padres, ahora, quién se la suministrará?

Extender el derecho a opinar a la vida diaria

Son muy importantes los pasos que se están dando para consolidar las conquistas de la Revolución y ratificar su carácter socialista. Las palabras de nuestros máximos dirigentes apuntan a desechar viejas fórmulas y normas que, en los momentos actuales, entorpecen y lastran la economía y la vida en general. El reunionismo y la pasividad de muchos funcionarios son visibles ante los ojos de todos y, a pesar de los llamamientos al cambio, la mayor parte del tiempo se asiste para escuchar las mismas palabras que en otras ocasiones y los mismos compromisos incumplidos.

Las palabras del compañero Machado Ventura apuntan a erradicar de una vez estos males.

Las cosas van cambiando en nuestra bloqueada isla y la gente lo percibe en las calles, aunque no sea al ritmo y manera que muchos añoramos.

Nuestro Presidente, Raúl Castro, en el recién concluido Segundo Pleno del Comité Central del Partido, fue enfático al señalar que cualquier discrepancia existente dentro de la Comisión Permanente de Implementación y Desarrollo será analizada sin desecharla de golpe y con ello garantizar la libre discusión para que cada quien opine lo que piense y desee. Como dijo después en la Asamblea Nacional, se hacen extensibles estas palabras a la vida diaria del cubano para que este tenga un espacio donde ejercer su derecho a opinar con las palabras adecuadas y el debido respeto.

Me uno a aquellos que piden que nuestras Mesas Redondas se nutran de las palabras y opiniones de los temas nacionales.

Hay personas con muchas cosas que decir, con muchas ideas que aportar, que merecen y deben ser escuchadas. No debemos temer al señalamiento justo o a la crítica acertada, siempre que vengan con un espíritu constructivo, con educación y si aportan soluciones, pues mucho mejor.

Urge crear un mecanismo que no tenga otro comprometimiento que con la verdad y la justicia, para hacer llegar a todos los niveles necesarios los reclamos que hace a diario la población sobre tanto maltrato que institucionalmente sufren por parte de funcionarios que ocupan cargos creados para servir al pueblo. Lamentablemente, y lo digo por experiencia propia, una enorme parte de las quejas que se hacen se pierden en la complacencia y la indolencia. Causa rabia y a la vez dolor que casos publicados en la prensa nacional tengan una pronta respuesta luego de aparecer en estos medios, a pesar de que los remitentes anduvieron y desandaron durante meses, y a veces años, en busca de una solución a sus problemas. La mayor parte del tiempo, los jefes se daban por enterado de los casos (que muchas veces esperaban por una firma de ellos mismos), solo cuando toda Cuba ya lo sabe. Lamentablemente, hay muchos cuadros que se "cuadran" ante situaciones cuya respuesta aún no baja por la canalita.

Todos podemos contribuir con lo que el país y el pueblo necesitan en estos momentos. Todos debemos ser escuchados. A veces las trincheras de ideas valen más que las de piedra.

¿Acabaremos las indisciplinas o no?

Son más de dos años leyendo opiniones acerca de cómo lograr mejoras y transformar lo que nos hace daño en opciones que reimpulsen nuestra economía para el bien de todos y se rescaten los objetivos principales por los cuales luchamos en diferentes formas y lugares y que no vemos cuajar con la prontitud y calidad deseadas. Es un arrastre de años.

La credibilidad popular sobre algunas instituciones y funcionarios es cada vez más endeble, con tendencia alarmante, y hay que revertirla.

Es necesario llevar un inventario perpetuo de los problemas (Ahí están los diferentes ministerios y el Poder Popular) y acorralar cualquier desvío moral (moral encierra toda clase de desvíos y cosas mal hechas), pues no podemos esperar por las calendas griegas o enterarnos en congresos, asambleas, reuniones y lo que es peor, en algún comentario aflorado por la prensa nuestra o mucho peor, la extranjera.

Al igual que en un almacén, no es difícil instrumentarlo y a su vez caerle con toda la carga necesaria (como pedía Villena) para liquidarlo al nacer ¿Hasta cuándo?

El cuadro que no pueda asumir esto y responder no puede estar ahí (sea el nivel que sea), alguien tiene que poder o si no de qué soluciones estamos hablando.

¿Acabaremos las indisciplinas o no?

Cada ministerio sabe, pues hay leyes y resoluciones que se legislaron para bien, qué se debe hacer y qué no. El que no domine esto en la estructura vertical de dirección y la horizontal está fuera de sintonía y por tanto seguirá el relajo. Cuando algo se considera obsoleto, al igual que los bienes en desuso (como las piezas de repuesto o insumos) deslindarlo del listado de los que tienen utilidad y depurar.

Así hacer con las barbaridades latentes que hacen mucho daño. Darle continuidad es el factor esencial, perpetuo, sí, día a día. ¿Cómo? ¿Y los cuadros y personal administrativo serios? Pero cuidado. Que la escalera de dirección no tenga ningún eslabón podrido. Eso contagia y entonces sí que el buen deseo se va a bolina. ¿Tendremos lo que tenemos que tener?

Quien no pueda, no debe seguir. En el pueblo hay gente seria y capacitada y con deseos para afrontar tareas y sacrificarse.

Tenemos que salvar el socialismo. Tenemos que corresponder a Fidel y Raúl.

¿Realmente todos los llamados cuadros, sean políticos o administrativos, son cuadros de verdad? Pienso que muchos están haciendo sombra y defendiendo subrepticiamente posiciones oportunistas y por tanto, simple y llanamente se tienen que ir. El que haga sombra se va. ¿Adónde?, adonde aprenda o vaya a pagar la desidia o negligencia, que seguro eso se traduce en un mal intencionado actual o negligencia adquirida.

Por último, no desestimar (en los inventarios) los cargos simples a nivel operativo donde primero se piensa en qué me puedo buscar y el sueldo le resulta como algo segundario, pues es mucho menor que el resultado del desvío, llámese también robo.

Es raro encontrar un puesto de servicio o producción en que no se den estas cosas. Esta es una Revolución de los humildes, con los humildes y para los humildes.

El Presidente Raúl ya ha tomado algunas medidas y tiene puesto más el pie en el acelerador que en el freno y su cuerpo de apoyo (a todos los niveles) debe empinarse y ascender al pueblo más todavía. Solo así chocaremos con la verdad. Es grande la tarea, pero debemos recordar siempre el encuentro de 5 palmas.

Mejoremos el servicio de salud con una mejor actitud, nosotros podemos

Conversando, hace varios años, con un amigo médico del hospital 10 de Octubre (más conocido por La Dependiente) me dice que vaya a verlo a su consulta, pero que lo haga sobre las 12 del día y seguido me explica que a esa hora ha terminado de atender a los pacientes que fueron citados o tenían turno con él. Han pasado muchos años y cada año se ha llevado un poco de la disciplina que debe caracterizar el acceso a una consulta médica. Ese médico consideraba que sus amistades no podían pasar por encima o delante de todas esas personas que llegaron temprano a la consulta. Además, tenía en cuenta que sus pacientes también son parte de sus amistades, él los respeta y también ellos sienten respeto y consideración hacia su persona.

Este comentario me da pie para señalar que las colas en las consultas médicas, tanto en el consultorio como el policlínico y el hospital, se han convertido en motivo de desorden, donde no pocas veces prolifera el amiguismo y donde adelantan en la cola los que traen los mejores obsequios para los médicos y demás. Adelanto que no estoy en contra de estimular al médico y a la enfermera, pero no pueden marcar diferencia en el orden para la atención a los pacientes.

En cualquier especialidad y en cualquier parte se desata el desorden porque en primer lugar esa enfermera que trabaja con el médico o la secretaria de la sala no asume la responsabilidad de anotar a los pacientes según van arribando a la consulta e ir indicando a cada uno que debe esperar a ser llamado. Hay consultas donde un cartel indica: "Espere ser llamado, el médico da la prioridad". No estoy en desacuerdo, pero la prioridad para los amigos y otros conocidos le quitan autoridad al médico cuando quiere dar respuesta al reclamo de los que son verdaderos pacientes, a los que saben esperar su turno.

Todo el que va a un hospital es porque tiene alguna dolencia. No va por gusto. Sabemos bien que vamos directo al cuerpo de guardia del policlínico o del hospital porque a veces no hay médico en el consultorio o el médico llega a cualquier hora y se marcha temprano por disímiles razones, ciertas o no. A veces llega tarde porque el municipio le citó para una reunión u otra tarea ajena a la que tiene en el consultorio que desde su origen está para atender a las familias. Por ello el compañero Fidel lo identificó como el consultorio del médico de las 120 familias. Hoy, en realidad, son mucho más de 120 familias para su atención. En estos casos, el médico no tiene culpa de que lo ocupen en otra cosa y se siente mal cuando escucha las quejas de la población bajo su atención porque la respuesta que tiene es cuestionable.

Por otro lado, como por arte de magia, llegan los medicamentos por la mañana a la farmacia y temprano en la tarde ya no hay muchos de ellos. ¡Qué misterio! se desaparecen y otros los acaparan algunos inescrupulosos e impunes comerciantes sin licencia para tal actividad.

Puedo añadir algunos señalamientos:

—La disciplina y la organización en la consulta son ta-reas del médico, la enfermera y la secretaria de la sala o especialidad. Esta es opinión pública que se escucha diariamente. Por desgracia, con mucha frecuencia asisto a consultas y he tenido la oportunidad para identificar lo que es bueno y lo que no funciona bien.

—No se entiende, ni hay explicación, de por qué faltan las recetas en las consultas. A veces se le entrega a un médico 10, 20 ó 30 recetas, como si alguien pudiera planificar la cantidad de pacientes que van a necesitar un medicamento.

—Pocos médicos realizan exámenes clínicos a sus pacientes y en muchos casos, sin pensarlo dos veces, le indican TAC o Resonancia o la indicación más simple donde sin apenas escuchar al paciente le indican un medicamento, retornando el paciente en peor estado y ellos ya no están. Esto, muchas veces ocurre, porque estaban conversando por teléfono o personalmente con un amigo o una amiga y la conversación tenía prioridad. Falta ética médica en algunos galenos.

—Algo que ocurre con mucha frecuencia: el médico tiene a varias personas en la consulta y uno entra pensando que puede hablar o contestar preguntas privadas, asuntos muy privados y uno se va sin decir al médico la verdad o evade determinadas preguntas. La privacidad en la consulta la tiene que garantizar el médico.

—En muchos lugares es urgente crearles mejores condiciones de trabajo a los médicos, mejorar la alimentación o que trabajen una jornada corrida.

—Un médico que pasa una noche de guardia no está en condiciones de por la mañana seguir trabajando ni atender a ningún tipo de paciente. Los he visto atendiendo a niños y bostezando. ¿Quién puede asegurar que se concentró bien en el estado de salud de ese niño? ¿Así se puede asegurar un buen diagnóstico?

—Es raro ver a un consultorio médico trabajar por las tardes. De todas partes los pacientes son remitidos a sus consultorios como si estuvieran abiertos con el médico cualquier día y a toda hora laborable.

—Si no atendemos a tiempo las condiciones físicas de estas instalaciones de la atención primaria de salud menos posibilidades de reparación tendremos mañana porque los gastos serían superiores y nuestra economía no aguanta más carga.

Se sabe que el MINSAP está trabajando por revertir tal situación y que se hace el esfuerzo por aumentar la calidad de la atención al pueblo. En la Mesa Redonda de la semana pasada se pudo apreciar todo lo que se está haciendo en el país por mejorar el servicio de salud.

En muchos lugares del mundo están todos los recursos posibles y el personal más calificado; sin embargo, no alcanzan nuestros niveles de asistencia médica, porque allí faltan corazones, solo se priorizan intereses.

Para Cuba, en nuestras condiciones económicas y otras carencias, esto nos indica y sabemos bien, que cuando el factor humano está priorizado, bien atendido, tanto el que presta la atención al paciente como el propio paciente cuando la recibe y reconoce en ello calidad e interés de alegrar corazones, vale más que todo el dinero del mundo. Aquí, en la salud, no se trabaja por dinero y no se ignora que el salario no alcanza. Una sonrisa siempre llega al corazón.

De nada sirve una institución de salud bien equipada si no se trabaja con el único deseo de contribuir al mejoramiento de la salud humana. No permitamos que caiga nuestra reconocida condición de potencia médica mundial. Mejoremos el servicio de salud con una mejor actitud, nosotros podemos y lo sabemos.
