Production of Cuban Medicines and Reduction of Imports

Only in the first semester of 2009, by way of the reduction of medicinal imports, the Quimefa Group of Pharmaceutical Companies has saved the country 1.4 million dollars. But this is not the only benefit obtained: the increase of availability for the people is another invaluable satisfaction.

TALENT AND DETERMINATION

Qimefa’s director for Quality and Development, Ramón Arango, offers this data and explained that the Basic Table of Medicaments (composed in 2009 of 562 national and 307 imported medicines) constitutes the main reference in the effort to replace drugs purchased abroad with similar products obtained in Cuba.

The centers of Havana’s Scientific Complex have joined the Group in this endeavor, with generic medicaments, vaccines and other biotechnological products.

Talent and determination are aimed at the 307 medicines that up till now are necessary to be bought overseas. They also include others that are new.  All of them are studied so as to determine their therapeutic importance, economic effectiveness and economy of scale.

Each drug is submitted to professional testing. Likewise, assessments of whether it’s cheaper to produce them than to import them are made.  Sometimes, even though it could be a little bit more expensive or yield no economic benefits, the decision to produce it is based on its therapeutic importance, and even on its strategic importance. The other requirement is for its production to be justified by demand.

If these two requirements are met, the mechanism is started, and, when the research work bears fruit, the difference between cost and the price in the international market is even more perceptible.

THE PEOPLE COME FIRST

One of the businesses yielding the largest profits on the planet is that of medicaments. They take advantage of the fact that their products can’t be scorned by patients, comments Arango. "Research work is expensive and entails long periods of study; however, costs usually don’t bear relation to price".

In Cuba, when we manage to replace a medicament, we can commercialize it at a reasonable price, according to its real production cost, adds the Quimefa executive. Many times, the State assumes subsidies, in order not to affect the population.

"In addition, we not only cover the volume we used to introduce into the country, but also multiply it: each 5 ml bottle of Hypertonic Sodium Chloride –drops for eye problems- represented an expenditure of two dollars. For Cuba to produce it, it is barely 30 cents. Now, patients have five times more bottles available, as compared to the 5,000 they used to have."

The director for Investments of the Roberto Escudero Pharmaceutical Laboratory, Vladimir Díaz, affirms: "In the world, this industry is a gold mine, a privatized business with the sole intention of obtaining profits. Cuba, in turn, continues to commit itself to look after the population, sparing no expense. The new line installed in our center involved the investment of over one million dollars. This, of course, guarantees quality and speed, and doubles production".

This laboratory is the only one devoted to creams, ointments, pessaries and suppositories, from San Antonio to Maisí, that is, from one end of the island to the other. Between its two lines, it produces 50,000 tubes a day.  At the time of our visit, the brigade made up by Caridad, Jenny, Beatriz, Raúl, Daisy, Yordanka, José Antonio and Felipe, was working hard to meet the demand of Miconazol.

SAVING MILLIONS

The usual source of riches for capitalists is turned into –by the grace of a system that thinks about each of its members- a mine that saves everyone’s gold.  According to Ramón Arango, the plan for the 2007-2012 period incorporates 73 medicaments, 31 of which replace imports and 42 are added to the Basic Table, being new.

They include different pharmacological groups, like antibiotics and drugs of ophthalmic, cardiovascular and skin use, as well as anti-inflammatory, anti-asthmatic and anti-parasite products.

During 2008, the six products produced by Quimefa allowed for a 1.1 million dollar saving, while, during the first semester of 2009, with another 10 that were not necessary to import, the expenditure of 1.4 million dollars was avoided.

"The main objective of the research and development process is to replace imports, with which we save hard currency and increase the availability of medicaments for the population. At the same time, we update the Basic Table (with drugs that are more effective from the therapeutic point of view) and we are expanding the list of items the country can export.

"In the case of exports, we’re faithful to the premise of not commercializing a product that might be in short supply in the national network."

96% COVERAGE

With respect to the internal market, the Quimefa Group representative said that their objective is to carry out supplies based on studies of demand, taking morbidity into account and on the basis of rational use, not following a set plan, since sometimes there are unpredictable situations, such as the AH1N1 pandemic.

"Raw material and resources are not always within reach, a phenomenon intensified by the US blockade and the effects of the world economic crisis, besides the terms of copyright, which also exert a marked influence on the issue.  These and other factors of a production nature determine that, at a given moment, a medicament can run short. However, there’s over 96% coverage

"Not even the United States can be self-sufficient in medicaments or raw materials to produce them. Our country began a program to replace medical imports in 1991; since then, we have introduced some 400, by way of talent and efforts in the fields of research and development and the priority given by the State in terms of required financing".

The will and intelligence combine energies. The Center for the Research and Development of Medicaments (CIDEM), the Center of Engineering and Chemical Research (CIIQ), pharmaceutical laboratories, and the staff trained over the last 50 years are working for two beneficiaries who in fact are one and the same: the State... and the people.

Para importar menos medicamentos

Solo en el primer semestre del año, mediante la sustitución de importaciones de medicamentos, el Grupo Empresarial Farmacéutico Quimefa ha ahorrado al país 1,4 millones de dólares. Pero ese no es el único beneficio conseguido: el aumento de la disponibilidad para el pueblo entraña otra invaluable satisfacción.

Mediante el reemplazo de medicamentos adquiridos en el exterior por similares obtenidos en Cuba, la industria farmacéutica ha ahorrado al país 2,5 millones de dólares en dos años.

TALENTO Y EMPEÑO

Ramón Arango, director de Calidad y Desarrollo de Quimefa, ofrece estos datos y explica que el Cuadro Básico de Medicamentos (integrado en el año 2009 por 562 de producción nacional y 307 importados) constituye la principal referencia, en el afán de reemplazar los fármacos adquiridos en el exterior por similares obtenidos en Cuba.

Los centros del Polo Científico acompañan al grupo en el empeño, con medicamentos genéricos, vacunas y otros productos biotecnológicos.

Talento y tesón apuntan a los 307 medicamentos que hasta ahora es preciso comprar allende los mares. También incluyen otros que resultan novedosos. Todos son estudiados con tal de precisar su importancia terapéutica, efectividad económica y economía de escala.

El objetivo básico del proceso de investigación y desarrollo de medicamentos consiste en sustituir importaciones, con lo cual ahorrar divisas y aumentar la disponibilidad para la población.

Cada uno se somete a la comprobación profesional. Asimismo, se evalúa qué cuesta más, producirlo o importarlo; a veces, aunque sea un poco más costoso o no reporte ganancia alguna, la decisión de producirlo alude a su importancia terapéutica, e incluso estratégica. El otro requisito obedece a que la demanda lo justifique.

De cumplir con ambas exigencias, el mecanismo echa a andar. Y cuando la investigación rinde frutos, se torna aún más perceptible la diferencia entre el costo y el precio en el mercado internacional.

EL PUEBLO PRIMERO

La de los medicamentos es una de las empresas de mayor beneficio económico en el planeta. Sacan partido de que sus productos no pueden ser desdeñados por los pacientes, expresa Arango. "Las investigaciones son costosas, conllevan largos periodos de estudio; sin embargo, usualmente el costo no guarda relación con el precio".

Cuando en Cuba logramos sustituir un medicamento, podemos llevarlo a su costo real y comercializarlo a un precio razonable, añade el directivo de Quimefa. En muchas ocasiones, el Estado asume subsidios, a fin de no afectar a la población.

"Además, no solo cubrimos el volumen que introducíamos en el país, sino que lo multiplicamos: cada frasco de cinco mililitros de Cloruro de Sodio Hipertónico —un colirio para afecciones de la vista— suponía la erogación de dos dólares; producirlo nosotros, apenas 30 centavos. Ahora los pacientes disponen de cinco veces más frascos que los 5 000 de antaño."

Vladimir Díaz, director de Inversiones del laboratorio farmacéutico Roberto Escudero, sostiene: "En el mundo esta industria es una mina de oro, un negocio privatizado con la mera intención de obtener ganancia. En cambio, Cuba sigue apostando por atender al pueblo, sin reparar en cuán costoso sea. La nueva línea instalada en nuestro centro implicó invertir más de un millón de dólares; desde luego, garantiza calidad, rapidez y duplica la producción".

Este laboratorio es el único dedicado a cremas, ungüentos, óvulos y supositorios, desde San Antonio a Maisí. Entre sus dos líneas logran 50 000 tubos diarios. Al momento de nuestra visita, la brigada compuesta por Caridad, Jenny, Beatriz, Raúl, Daisy, Yordanka, José Antonio y Felipe, laboraba con tal de cubrir la demanda de Miconazol.

AHORRO DE MILLONES

La usual fuente de riquezas para los capitalistas, se transforma —por obra y gracia de un sistema que piensa en cada uno de sus miembros— en una mina que ahorra el oro de todos.

De acuerdo con Ramón Arango, el programa previsto para el periodo entre el año 2007 y el 2012 incorpora 73 medicamentos, de los cuales 31 sustituyen importaciones y 42 se introducen al cuadro básico por resultar novedosos.

Abarcan diferentes grupos farmacológicos como antibióticos, oftálmicos, cardiovasculares y dérmicos; también antinflamatorios, antiasmáticos y antiparasitarios.

Durante el año 2008, los seis productos logrados por Quimefa permitieron el ahorro de 1,1 millones de dólares. Mientras, en el primer semestre del 2009, con otros 10 que dejó de ser necesario importar, se evitó la erogación de 1,4 millones de dólares.

"El objetivo básico del proceso de investigación y desarrollo de medicamentos consiste en reemplazar importaciones, con lo cual ahorramos divisas y aumenta la disponibilidad para la población. A la par, actualizamos el cuadro básico (con fármacos más efectivos terapéuticamente) y ampliamos la cartera de productos para exportar.

"En el caso de las exportaciones, somos fieles a la premisa de no comerciar ninguno que pudiera faltar en la red nacional."

COBERTURA AL 96%

Sobre el mercado interno, el representante del grupo Quimefa alegó que trabajan con el propósito de abastecer, no por plan (pues a veces existen situaciones imprevisibles, como la pandemia AH1N1), sino a partir de estudios de demanda, tomando en cuenta la morbilidad y en base al principio de uso racional.

"No siempre las materias primas y los recursos están al alcance de la mano, fenómeno agudizado por el bloqueo y los efectos de la crisis económica mundial, además de los términos de propiedad intelectual que ejercen una marcada influencia. Estos factores y otros de carácter productivo, condicionan que en un momento determinado pueda escasear un medicamento. Sin embargo, la cobertura supera el 96%.

"Ni siquiera Estados Unidos puede autoabastecerse del total de los medicamentos o materias primas para producirlos. Nuestro país inició el programa de sustitución de importaciones de fármacos en 1991; desde entonces, hemos introducido unos 400, con talento y esfuerzo en la investigación y desarrollo, y la prioridad concedida por el Estado mediante el financiamiento requerido".

La voluntad y la inteligencia juntan energías. El Centro de Investigación y Desarrollo de los Medicamentos (CIDEM), el Centro de Ingeniería e Investigación Química (CIIQ), los laboratorios farmacéuticos, todo el capital humano formado en 50 años trabajan para dos beneficiarios que son uno solo: el Estado... y el pueblo.
