Hypnosis in sex therapy

Its techniques are applied successfully in Cuba to resolve partner conflicts and to help people with HIV or those who have suffered violent sexual experiences

SANTIAGO DE CUBA .  Every human being is the result of their life history.  We react to any obstacle in a conscious or unconscious manner mobilizing psychological resources acquired over the years.

However, many people underestimate the power that those emotional elements have to take us from a state of physical health to one of disease, or vice versa.


This also happens with sexual dysfunctions: when they are not attended to on time, they generate depression, anxiety, low self-esteem, and ultimately affect the erotic life of the people involved.

Options for addressing this type of problem are many, but people do not always know where to find them or are not encouraged to ask for professional help.

This is observed by Joaquín Blanco, Masters in Psychology, professor at Oriente Universite and one of the promoters of hypnosis therapy in this city. It is a technique highly regarded throughout the world for the speed of its effectiveness, because it promotes the internal resources that have served the person and helps to put them into effect. This is helpful in order for the individual to face new conflicts or process past traumas, in light of the psychological tools with which they relate to now.


The City of Havana, Camagüey and Santiago de Cuba are provinces within the country with solid results in the application of these techniques.  This last session of the biannual National Congress of Hypnosis (begun in 1999) is where we interviewed Dr. Blanco. Additionally, courses are offered at this congress on the topic of scientific events like the Third International Workshop of Psychology, which took place last April.


The first thing is willingness

In the matter of health, worry resolves nothing, dealing with it resolves it, Blanco recalls, but he recognizes that many external factors condition the search for help when the malaise involves sexual performance.


Some people still reject consultations at their polyclinics for fear of what they will say, but nowadays it is seen as a major assistance by the population who know of these services, he says.  "In many cases the impetus comes from the other member of the couple, who conditions the continuity of the relationship on the acceptance of help.

"For men it is harder to deny the difficulty because it is more visible: a premature ejaculation or an erectile dysfunction is expressed very quickly and generates immediate displeasure in the partner.  On the other hand, a woman can have a dysfunctional response which for her, internally, is negative but she may not externalize it and choose to feign orgasm or hide vaginal pain", he explains.


For that reason, he insists that the success of hypnosis depends more on the willingness of the patient than on the skill of the hypnotist, who, in the long run, is a facilitator:  "one who mobilizes the resources of the other person so this breaks their rational resistance and allows them to access the subconscious, but it is the subject who makes the suggested changes".

As Blanco explains, the therapist does not put anything new into the patient, they are their own fantasies and memories, which through hypnosis lead them to represent or perceive sensations which were assumed lost, and to rescue the good moments of their day to day life.

This technique is often used to counteract regulatory tensions in men whose ejaculation is early or delayed, because it reduces anxiety and leaves him free to experience his orgasm in an efficient, healthy, natural manner, far from those myths which promote a prolonged erection in order to "satisfy?" a woman, which in the long run ends in a stressful excess of control of the ejaculatory process and in an amorous disaster.


Another disorder treated by this method is erectile dysfunction (ED): in the hypnotic state the subject is offered symbols which help him to represent the situation in a more optimistic manner, for example, through the image of a crane capable of strongly raising its arm and maintaining itself rigid and straight for the necessary time, according to Blanco.

But not always do conflicts in the area of sexuality respond to the erotic moment by itself:  often they reflect problems of another kind, such as isolation.

Hypnosis can help to program those interactions in a more constructive form, revealing unconscious sabotaging attitudes by the subject or partner (such as emotional blackmail) and showing how these reactions generate low self-esteem in both.


The purpose of therapy in these cases is to work for self-acceptance, self-esteem, but also empathy: to teach them to put themselves in the place of the other person and to listen like they would want to be heard.

Another facet of sexual life in which hypnosis is a lot of help is overcoming the aftereffects of abuse or sexual violation. "It is a process that takes time, especially if the trauma is far in the past, but working with these cases in our country has borne fruit."

As Blanco explains, traumatic events are recorded in the unconscious and are then experienced according to the age and the psychological resources of the victim when the incident occurred.  Through the techniques of regression to the past and projections to the present success is achieved by the resources with which the person has today, their maturity and their capacity of response today.


Thus, phobias (irrational fear of any element) are overcome, which in sexually-assaulted women can be generating sexual dysfunctions in varying degrees:  lack of desire, rejection of intimate contact, anorgasmia, vaginismus (the vagina cannot be penetrated) or dispareunia (pain during intercourse).

One is not trying to make them live that anguish again, the expert clarifies, but to regain positive feelings, prior to and after the fact, while their resources show them how to face difficult moments and help them to move from an unhealthy state toward better health, always with positive suggestions to appraise that conflict differently and to improve their interpersonal relationships.


Clearly, if the person does not do their part, it is impossible to achieve significant changes in their lives.  Since sometimes the patients who come to consultation obtain secondary benefits from the unresolved conflict —whether they are conscious or not of that fact— they are not interested in confronting it.

For that reason, the ethical commitment of the therapist is important, which ranges from achieving a good initial interview, to controlling the suffering that is intended to work from the hypnosis, to knowing their limits, and above all, to having a holistic orientation, integrating psychotherapy, so that it allows them to open themselves to a therapeutic combination with other natural pathways such as acupuncture or homeopathy.  "We cannot believe that hypnosis, or any other method, is a total panacea for human conflicts, but in our country it has good prospects for use", he concluded.

Caribbean Hypnosis

In Santiago de Cuba hypnosis therapy has three centers of reference: Joa
quín Duany military hospital, Saturnino Lora provincial hospital and John Bruno Zayas Surgical and Gynecobstetrical Clinic.  In addition, multidisciplinary consultation is offered in several polyclinics, which include specialists in gynecology, endocrinology, urology, psychology and psychiatry.

As many young people as adults come and from both sexes.  "In the beginning we focused on male dysfunctions but now we also treat females and the service has great acceptance.  We have gradually incorporated hypnosis treatment for depression created by loneliness, widowhood, and personal and familial acceptance of homosexuality", he adds.

When there are no underlying clinical problems, more than 80 percent of cases resolve their conflicts in a short time, with an average of nine weeks of treatment.

Hypnosis is also used in the sexual rehabilitation of patients who have received a penile prosthesis and to help overcome the fear of death and to improve the immune system of people living with HIV/AIDS.

Ask up front

A.A.:  For nine months I have had a relationship with a boy.  In the past two months he hasn't reached orgasm.  He says that he feels a lot of pleasure being with me. This happened with another relationship and after some time he left it.  Never was it dealt with.  It worries both of us, because we have a life planned, in addition to loving each other a lot.

What is called delayed ejaculation occurs in men, which consists of a delayed or absent orgasm.  When the reason is psychological, he is in the habit of controlling himself in excess and, unconsciously, his ejaculation.  Some medications and drugs can alter ejaculation.  Diabetes also can cause this disorder.

If it has already happened to him and it evolved favorably without specialized help, it is possible that now it will fare likewise.  But you should not be afraid to consult a specialist if the problem persists.  This will help you to figure out the cause and solve the problem.

If he is not consuming some toxic substance and his health is good, it would be advisable to assess if something in the relationship is not working smoothly.  His orgasmic difficulty could appear to signal that something does not proceed as desired, although it is very likely that he does not know it very clearly.

Having a planned life and loving each other a lot can act as a obstacle to recognizing any difficulty that is occurring.  One thinks that it is not possible that something goes wrong, but, in truth, contradictions are a part of life, and a couple is no exception.

For that reason, it would be good if you helped him to talk about what is going through his mind during these times, beyond what seems trivial or significant to him.

Sometimes one is afraid to recognize conflicts as if they were a Pandora's box. It is also true that we need to know how to face them with sanity.  It is precisely these challenges that strengthen the bond every time they are surmounted.

SANTIAGO DE CUBA.— Cada ser humano es el resultado de su historia de vida. Ante cualquier obstáculo reaccionamos de manera consciente o inconsciente movilizando los recursos psicológicos adquiridos a lo largo de los años.

Sin embargo, mucha gente menosprecia el poder que tienen esos elementos emocionales para llevarnos de un estado de salud física a otro de enfermedad, o viceversa.

Así pasa con las disfunciones sexuales: cuando no se atienden a tiempo generan depresión, ansiedad, baja autoestima, y a la larga afectan la vida erótica de las personas involucradas.

Opciones para enfrentar este tipo de problemas hay muchas, pero no siempre la gente sabe dónde hallarlas o no se anima a pedir ayuda profesional.

Así lo percibe el máster Joaquín Blanco, psicólogo, profesor de la Universidad de Oriente y uno de los promotores en esta ciudad de la terapia por hipnosis, vía muy apreciada en todo el mundo por la rapidez de su efectividad, pues promueve los recursos internos de los que se ha servido la persona y ayuda a actualizarlos, para enfrentar nuevos conflictos o procesar traumas del pasado del individuo, a la luz de las herramientas psicológicas con que cuenta ahora.

Ciudad de La Habana, Camagüey y Santiago de Cuba son provincias con sólidos resultados en la aplicación de estas técnicas en el país. En esta última sesiona cada dos años el Congreso Nacional de Hipnosis (comenzó en 1999) y además se ofrecen cursos sobre el tema en eventos científicos como el III Taller Internacional de Psicología, realizado el pasado abril, donde entrevistamos al doctor Blanco.

Lo primero es la voluntad

En materia de salud la preocupación no resuelve nada, lo que resuelve es ocuparse, recuerda Blanco, pero reconoce que muchos elementos externos condicionan la búsqueda de ayuda cuando el malestar involucra el desempeño sexual.

Algunas personas aún rechazan las consultas en sus policlínicos por temor al qué dirán, pero actualmente se percibe una mayor asistencia de la población que conoce de estos servicios, afirma. «En muchos casos el impulso viene del otro miembro de la pareja, que condiciona la continuidad de la relación a la aceptación de ayuda.

«Para los hombres es más difícil negar el conflicto porque es más visible: una eyaculación precoz o una disfunción eréctil se expresan muy rápidamente y generan disgusto inmediato en la pareja. En cambio una mujer puede tener una respuesta disfuncional que para ella en su interior es negativa pero puede no exteriorizarla y optar por fingir el orgasmo u ocultar los dolores vaginales», explica.

Por eso, insiste en que el éxito de la hipnosis depende más de la voluntad del paciente que de la pericia del hipnólogo, que a la larga es un facilitador: «un ser que moviliza los recursos de la otra persona si esta rompe su resistencia racional y le permite acceder a su subconsciente, pero es el propio sujeto quien realiza los cambios sugeridos».

Según aclara Blanco, el terapeuta no pone nada nuevo en el paciente, son sus propias fantasías y sus recuerdos, los que a través de la hipnosis le llevan a representar o percibir sensaciones que daba por perdidas, y a rescatar los buenos momentos de su cotidianidad.

Esta técnica se emplea mucho para contrarrestar tensiones regulatorias en hombres cuya eyaculación resulta precoz o retardada, pues reduce su ansiedad y lo deja en libertad de vivenciar su orgasmo de una manera eficiente, saludable, natural, lejos de esos mitos que promueven una erección prolongada para ¿satisfacer? a la mujer, que a la larga terminan en un estresante exceso de control del proceso eyaculatorio y en un desastre amatorio.

Otro trastorno atendido por esta vía es la disfunción eréctil (DE): en estado hipnótico se le ofrecen al sujeto símbolos que le ayuden a representar la situación de manera más optimista, por ejemplo a través de la imagen de una grúa capaz de levantar fuertemente su brazo y mantenerse rígida y recta el tiempo necesario, según cuenta Blanco.

Pero no siempre los conflictos en la esfera de la sexualidad responden al momento erótico en sí: muchas veces reflejan problemas de otra índole, como la incomunicación.

La hipnosis puede ayudar a programar esas interacciones de una forma más constructiva, revelar actitudes saboteadoras no conscientes del sujeto o de su pareja (como los chantajes emocionales) y mostrar cómo estas reacciones generan una baja autoestima en ambos.

El propósito de la terapia en estos casos es trabajar la aceptación de sí mismo, la autoestima, pero también la empatía: enseñarles a ponerse en el lugar de la otra persona y a escuchar cómo quisieran ser escuchados.

Otra faceta de la vida sexual en la que la hipnosis resulta de mucha ayuda es la superación de secuelas de un abuso o una violación sexual. «Es un proceder que lleva tiempo, sobre todo si el trauma es lejano en el tiempo, pero el trabajo con estos casos en nuestro país ha dado frutos».

Según explica Blanco, las experiencias traumáticas se graban en el inconsciente y se vivencian luego según la edad y los recursos psicológicos de la víctima en el momento en que el hecho ocurrió. A través de técnicas de regresión al pasado y proyecciones al presente se actualiza el suceso a la luz de los recursos con que la persona cuenta hoy, su madurez y su capacidad de respuesta actual.

Así se logran vencer fobias (miedo irracional a cualquier elemento) que en mujeres violentadas sexualmente pueden estar generando disfunciones sexuales en diferentes grados: falta de deseo, rechazo al contacto íntimo, anorgasmia, vaginismo (la vagina no se deja penetrar) o dispaurenia (dolor durante el coito).

No se trata de hacerles vivir aquella angustia de nuevo, aclara el experto, sino de recuperar sensaciones positivas previas o posteriores al hecho que le muestren sus recursos para enfrentar momentos difíciles y le ayuden a transitar de un estado insano hacia uno de mejor salud, siempre con sugerencias positivas para valorar aquel conflicto de una manera diferente y mejorar sus relaciones interpersonales.

Claro que si la persona no pone de su parte es imposible llegar a cambios significativos en su vida, y como a veces los pacientes que llegan a consulta obtienen ganancias secundarias del conflicto no resuelto —estén o no conscientes de ese hecho— no se interesan en enfrentarlo.

Por eso es importante el compromiso ético del terapeuta, que va desde lograr una buena entrevista inicial hasta dominar los padecimientos que se pretende trabajar desde la hipnosis, conocer sus límites, y sobre todo tener una mirada holística, integradora, de la psicoterapia, de modo que le permita abrirse a la combinación terapéutica con otras vías naturales como la acupuntura o la homeoterapia. «No podemos creer que la hipnosis, ni ningún otro método, son la panacea total para los conflictos humanos, pero en nuestro país tiene buenas perspectivas de uso», finalizó.
 
Hipnosis caribeña

En Santiago de Cuba la terapia por hipnosis tiene tres centros de referencia: el hospital militar Joaquín Duany, el hospital provincial Saturnino Lora y el Clínico Quirúrgico y Ginecobstétrico Juan Bruno Zayas. Además, se ofrece consulta multidisciplinaria en varios policlínicos, que involucra a especialistas en ginecología, endocrinología, urología, psicología y psiquiatría.

Vienen tanto personas jóvenes como adultas y de ambos sexos. «Al principio nos enfocamos en las disfunciones masculinas pero ahora tratamos también las femeninas y el servicio tiene gran aceptación. Hemos incorporado paulatinamente el tratamiento hipnótico de la depresión que genera la soledad, la viudez, y la aceptación personal y familiar de la homosexualidad», agrega.

Cuando no hay problemas clínicos de base, más del 80 por ciento de los casos resuelve sus conflictos en un plazo breve, con un promedio de nueve semanas de tratamiento.

La hipnosis se emplea también en la rehabilitación sexual de pacientes que han recibido una prótesis peneana y para ayudar a superar el miedo a la muerte y mejorar el sistema inmunológico de personas que viven con VIH/sida.

 

Pregunte sin pena

A.A: Hace nueve meses mantengo una relación con un muchacho. En los últimos dos meses él no llega al orgasmo. Afirma que siente mucho placer al estar conmigo. Esto le sucedió con otra relación y al pasar un tiempo se le quitó. Nunca se trató. A ambos nos preocupa, puesto que tenemos una vida planeada, además de querernos mucho.

En los hombres ocurre la llamada eyaculación retardada, consistente en la demora o ausencia del orgasmo. Cuando la razón es psicológica suele controlarse en exceso e inconscientemente la eyaculación. Algunos fármacos y drogas pueden alterar la eyaculación. La diabetes también puede producir este trastorno.

Si ya le sucedió y evolucionó favorablemente sin ayuda especializada, es posible que ahora suceda igual. Pero no deben temer consultar a un especialista si el problema persiste. Este te ayudaría a discriminar la causa y solucionar el problema.

En caso de que no esté consumiendo sustancia tóxica alguna y su salud esté bien, sería conveniente valorar si algo en la relación no funciona igual. Su dificultad orgásmica podría aparecer para señalar que algo no marcha como se desea, aunque es muy probable que él no lo tenga muy claro.

Tener una vida planeada y quererse mucho puede actuar como obstáculo para el reconocimiento de cualquier dificultad que esté aconteciendo. Uno piensa que no es posible que algo ande mal, pero en verdad las contradicciones son parte de la vida, y la pareja no es una excepción.

Por eso, estaría bien si lo ayudases a hablar de lo que está pasando por su mente en estos tiempos, más allá de que le parezca trivial o significativo.

A veces se teme reconocer conflictos cual si fueran caja de Pandora. Es cierto que además es preciso saber enfrentarlos con cordura. Son justamente estos retos los que fortalecen el vínculo cada vez que se vencen.
